It is strongly recommended that your church hold a permission form for each child or young person participating in any ministry. It can be edited to include specific situations such as leaving the church property, transporting children in cars and photo/video permission.
For any ministry where children and youth are attending (without their parent/carer), such as creche, youth group, Sunday school, a ‘Sign in sign out form’ (or comparable process) is required. This document shows when the child or young person enters the care of the church/ministry leaders, and then, when they return to the care of their parent/carer.
